COLONOSCOPY INSTRUCTIONS

 

Medications: Attached is a prescription for SUPREP BOWEL PREP KIT, take this prescription to you local pharmacy to be filled.

 

Start your SUPREP BOWEL PREP KIT and the clear liquid diet on:

You must be on a clear liquid diet the entire day. This includes water, clear fruit juices, jello, and soup broth. On the back of this paper we provided you a sample menu.

 

SPLITDOSE REGIMEN: (Both 6 ounces bottles are required for a complete prep)

 

STEP 1:

Start first 6 ounce bottle of SUPREP BOWEL PREP AT: __7pm__on ___________

Complete steps below 1-4 using (1) six ounce bottle:

 

 

 

 

 

 

 

 

STEP 2:

On the morning of your procedure Start the second 6 ounce bottle of SUPREP BOWEL PREP AT: __2am____on ___________. Complete steps below 1-4 using the other six ounce bottle:

 

 

 

 

 

 

 

 

You must finish drinking the final glass of water by 3:30am.

 

Remember nothing to eat or drink after your last glass of water except for your medications with as little water as possible.

The Day of the Colonoscopy:        Date: _________________

·        Take your morning medications with as little water as possible.

·        DO NOTtake your diabetic medication unless told to do so. Please check your blood sugar before coming in for your procedure. (Diabetic patients only)

·        DO NOT stop taking your blood thinner medication unless instructed to by Dr. Dave or the prescribing physician.

·        Please be on time for your appointment.

·        Leave all jewelry at home.

·        There must be someone available to drive you home following your procedures, because you will not be permitted to leave the facility on your own.

·        Plan to be at the endoscopy center or surgery center for a total of 2- 3 hours.

·        Your follow up appointment should be at the bottom of your envelope.

·        Your results will be given to you at your follow up appointment.

·        Any questions please call the office or surgery center we will be glad to answer any questions you might have.
